Important Information for Kindergarten Students

ATTENTION! Important information regarding Kindergarten entry!


FINAL DEADLINE: 12/30/2020


North Carolina General Statutes require the following for students entering Kindergarten(even during virtual learning):


  1. Completed health assessment (physical exam). The blue NC Health Assessment Transmittal Form is needed. This requires completion by an authorized medical provider (physician, physician assistant, nurse practitioner) and turned into the school.

  2. Current and up to date immunization record. Most students entering K will need three (3) immunizations prior to school entry.

Both health assessment form and immunization record must be turned into the school. Failure to provide the required documents will result in your student being unable to attend school. You may contact the school nurse if you have questions.
